Lichen Planus & Lupus
Oral Lichen Planus (OLP)
- Wickham's striae — fine white lacy lines, the hallmark of the reticular form (most common, usually asymptomatic).
- Erosive form — most symptomatic; potentially malignant (low risk).
- Histology: hyperkeratosis, saw-tooth rete ridges, basal cell degeneration, band-like T-lymphocyte infiltrate. DIF: shaggy fibrinogen at the BMZ.
Lupus Erythematosus
Butterfly (malar) rash in SLE; oral lesions may mimic lichen planus. The lupus band test shows a granular band of immunoglobulin at the BMZ; anti-dsDNA is specific for SLE.
Exam Tips ⭐
Wickham striae = lichen planus; reticular = most common; erosive = most symptomatic; butterfly rash + lupus band test = lupus.
