Impacted Third Molars
Classifications
- Winter's classification — angulation of the impacted tooth: mesioangular, vertical, horizontal, distoangular.
- Pell & Gregory — relation to the ramus (Class 1/2/3) and depth relative to the occlusal plane (Position A/B/C).
Frequency & Difficulty
- Mesioangular impaction is the most common and usually easiest.
- Distoangular is generally the most difficult to remove (path of withdrawal into the ramus).
Considerations
Proximity to the inferior alveolar nerve, indications for removal (pericoronitis, caries, cyst), and surgical technique (flap, bone removal, sectioning).
Exam Tips ⭐
Winter = angulation; Pell & Gregory = ramus + depth; mesioangular = most common; distoangular = most difficult.
