Periodontal Flap Surgeries
- Original Widman flap — uses an internal bevel incision to remove pocket lining.
- Modified Widman flap — the most commonly used access flap; conserves tissue, allows root debridement and close adaptation (no major bone removal).
- Kirkland flap — an access flap without thinning (for suprabony pockets).
- ENAP (Excisional New Attachment Procedure) — internal bevel to curette pocket epithelium.
- Apically positioned flap — repositions tissue apically to reduce pockets / for crown lengthening.
Goals
Access for root debridement, pocket reduction, regeneration, and creating maintainable architecture.
Exam Tips ⭐
Modified Widman = most common access flap; apically positioned flap = crown lengthening; internal bevel incision is key.
